Got stuck outside the map after using PD teleport

Used my Phantom Dagger teleport on an eye after it teleported away and got stuck outside the map (Crypt). Eventually fell off the map entirely trying to get back and lost everything.

This is simply unacceptable for a 1.0 release. Third time I lost my entire purple kit to game bugs, twice in Crypt, all three going through the geometry. Come on guys, this CANNOT happen. Either fix it or make the goblin bring my stuff back via insurance.
